There are multiple ways that you can order an appraisal with Davis Appraisal Associates, Inc. In any of the methods, we will need to have an Appraisal Order Form filled out. You can obtain an Appraisal Order Form from the comfort of your home by downloading the form (below), signing it, and either faxing it or mailing it to us.

Should you not have access to a printer, or are unable to download the document, simply telephone us at (410) 620-6585 and we will gladly mail you a copy of the Appraisal Order Form. In any case, you may feel free to contact us by telephone, mail, or email with your questions about your specific appraisal needs. We look forward to hearing from you.

›› The process of mailing your filled out Appraisal Order Form to us

It is always a good idea to either print out two copies of the form, or photo copy the filled out form for your own records. File one of the copies away for your own records. This way you will have a documented copy of the terms and conditions pertaining to your appraisal.

A copy of the Appraisal Order Form all filled should be mailed to:

Or you can fax it instead to us at (410) 620-6586. If you choose to fax the copy, of course, the original can be filed away for your records. We do respectfully ask that you send the document by fax in the highest resolution settings available to you.

In all cases, please make sure that the form is filled out completely. Any portions of the form not completely filled out will be either considered to be non-applicable and could cause a time delay in completing the assignment. We reserve the right to cancel or modify any order forms not completely filled out.
